Welcome to the Eastern Community Network of Specialized Care covering the counties of Hastings & Prince Edward, Lennox & Addington, Frontenac, Lanark, Leeds & Grenville, Renfrew, Ottawa-Carleton, Prescott-Russell and Stormont, Dundas & Glengarry.
The Eastern Region CNSC is a collaborative network of community agencies, mental health service providers, primary health care providers, developmental service professionals and educators working together to better coordinate access to specialized services, improve the way services are delivered and promote professional development through increased sharing of research and training. The following are some examples of how our Network is making a difference in the community:
- helping to link developmental service and mental health providers, networks and committees to improve cross-communication
- participating in case conferences to assist in the planning for individuals with complex needs
- the Network has over 30 videoconferencing sites across the region to connect individuals and service providers with specialists, meetings and training events located outside their region
- an annual education calendar is developed to provide educational opportunities to those working with individuals with a dual diagnosis so that they may enhance their knowledge and skills
- our Health Care Facilitators provide support and assistance to individuals, families, caregivers and agencies in accessing health care services for those with complex needs
- The Eastern Network is also the lead for the provincial French Language Resource which is aimed at improving access to care and services for Francophones from a variety of cultural backgrounds
- partnering with developmental service and mental health providers to address identified service gaps in the community
